ART: Early Results for Bilateral IMA in CABG Offer Some Promises, Some Concerns
One-year data from what the lead investigator characterized as one of the largest-ever randomized clinical trials of two CABG surgery techniques has found no differences in major clinical events between patients treated with either single internal mammary artery grafts or bilateral internal mammary arteries, but a higher rate of sternal wound reconstruction in the latter. Medium-term results of systematic off-pump coronary surgery performed by trainee surgeons [Original articles]
Conclusions: The results of this study reflect our unit's long experience of OPCAB surgery and that its successful re-engineering towards the systematic use of OPCAB was feasible. They also show that, in this context, teaching OPCAB surgery is safe in a non-selected cohort of patients, and that the medium-term outcomes of the patients operated on by trainee or expert surgeons are similar. Comparative study of traditional long incision vein harvesting and multiple incisions with small skin bridges in patients with coronary artery bypass grafting at King Abdullah University Hospital - Jordan
The objective of our study is to determine the difference in wound complication and infection rates between two saphenous vein harvesting techniques, long incision versus multiple short interrupted incisions (tunneling) for coronary artery bypass grafting at the King Abdullah University Hospital - Jordan. METHODS: Retrospectively we analyzed data from 1,050 consecutive elective coronary artery bypass procedures performed from May 5, 2003, to December 31, 2007, in our institution. Saphenectomy using traditional Long incision vein harvesting (Group 1) performed in six hundred and fifty patients (n=650), while saphenectomy using multiple incisions with small skin bridges - tunneling (Group 2) performed in four hundred patients (n=400). Saphenectomy performed by the cardiac surgery assistant o...... MORE...

Comparison of patency between radial artery and saphenous vein in a coronary artery bypass grafting post operative with return of the symptoms
MORE...

BAS/BSCR29 Apolipoprotein(a) impairs adaptive remodelling in human saphenous vein endothelial and smooth muscle cells
Apolipoprotein(a) (apo(a)), a unique glycoprotein component of plasma lipoprotein(a) (Lp(a)),exhibits resistance to classical lipid-lowering treatments. Coronary artery bypass grafting (CABG) is commonly used to bypass coronary arteries diseased by atherosclerosis, routinely using the saphenous vein (SV) as a conduit. Early after grafting the SV adapts to the arterial environment through re-endothelialisation, and increased motility of smooth muscle cells (SMC). A clinical association between Lp(a) and coronary artery disease is evident; however, its role in vein graft failure is less clear. Endothelial cells (EC) and SMC were cultured from the SV of patients undergoing CABG. The influence of apo(a) on cellular activity was examined by proliferation (cell counting), chemotaxis (modified Bo...... MORE...

Thrombosed abdominal aortic aneurysm associated with an extensively “shaggy” aorta repaired anatomically via a thoracoabdominal approach with supraceliac aortic clamping: Report of a case
Abstract  A 76-year-old man with a history of multiple laparotomies and severe coronary artery disease was referred to our hospital after the sudden development of pain and numbness in the lower extremities. Computed tomography showed a thrombosed abdominal aortic aneurysm and diffuse aortic atherosclerosis; compatible with a “shaggy aorta.” A good response to thrombolytic therapy permitted elective scheduling of abdominal aortic surgery after coronary artery bypass grafting. We operated via an extended left retroperitoneal approach through a thoracoabdominal incision. Epiaortic ultrasonography revealed that only the supraceliac aorta was free of mobile thrombi and had minimal plaque; we therefore placed a proximal aortic cross-clamp there. Anatomic aortic reconstructio...... MORE...

Coronary artery bypass surgery in a patient with Kartagener syndrome: a case report and literature review
Kartagener syndrome consists of congenital bronchiectasis, sinusitis, and total situs inversus in half of the patients. A patient diagnosed with Kartagener's syndrome was reffered to our department due to 3-vessel coronary disease. An off-pump coronary artery bypass operation was performed using both internal thoracic arteries and a saphenous vein graft. We performed a literature review for cases with Kartagener's syndrome, coronary surgery and dextrocardia. Although a few cases of dextrocardia were found in the literature, no case of Kartagener's syndrome was mentioned. (Source: Journal of Cardiothoracic Surgery)... MORE...

CABG, PCI, or medical management for multi-vessel coronary artery disease?
Source: Circulation Area: News Ten-year follow-up of a study comparing three management strategies for patients with multi-vessel coronary artery disease provides useful data on the relative merits of each approach, with none clearly superior for overall mortality.   While there is good evidence on the treatment of some sub-sets of patients with coronary artery disease, there is still debate over the best way to treat others, such as those patients who are stable with preserved function, but have multi-vessel disease. Options for such patients are optimum medical therapy (MT), percutaneous coronary intervention (PCI), or coronary artery bypass grafting (CABG), and the MASS-II study (Medicine, Angioplasty, or Surgery Study) was set up to compare outcomes for these three options. O...... MORE...

Controversies in Oral Antiplatelet Therapy in Patients Undergoing Aortocoronary Bypass Surgery [REVIEW]
Oral antiplatelet therapy with aspirin and clopidogrel is a major strategy to prevent thrombotic events in patients with acute coronary syndromes and patients undergoing percutaneous coronary interventions. Although this therapy is associated with both short- and long-term clinical efficacy, irreversible platelet inhibition and ischemic events associated with premature withdrawal especially in patients treated with drug-eluting stents constitute a particular limitation in patients undergoing surgery. The current review article is focused on the central role of platelets in the occurrence of ischemic events, the significance of antiplatelet therapy, the potential hazards of drug withdrawal in patients needing coronary artery bypass grafting, and future perspectives.
