Bispectral index (BIS) is used to measure the patient's level of consciousness under general anesthesia to prevent surgical awareness. Several factors have been found to alter the BIS reading without affecting the depth of anesthesia, however. Anesthesiologists at a university hospital in Saudi Arabia noticed a change in BIS readings that appeared to be associated with changes in the position of the patient, either head-down as for gynecologic procedures or head-up as for laparoscopic cholecystectomy. Positioning of the patient is an important step of the surgical process, providing the required access to the structures that must be visualized or manipulated during the surgical procedure. Risk of proximal colonic neoplasms in asymptomatic adults older than 50 years found to have distal hyperplastic polyps on routine colorectal cancer screening.
Conclusion: Routinely recommending a colonoscopy for every patient with distal HPs found only by screening flexible sigmoidoscopy is neither justified nor necessary. Nevertheless, further investigation (ie, colonoscopy) may be warranted in the aforementioned subgroups.

Surgical treatment of cystic neoplasms of the pancreas
CONCLUSIONS: The most common cystic neoplasms of the pancreas are serous and mucinous cysts. These tumors are more frequent in female. Although almost all serous cysts are benign, 40% of mucinous cysts are malign. Durability of the self-approximating translumenal access technique (STAT) for potential use in natural orifice translumenal surgery (NOTES)
Conclusions Although this study was a limited, prospective, animal survival study without a control arm, it again indicates that STAT
allows safe abdominal access, a reliable means of closure, and directed endoscope positioning. Although one significant mucosal
