Cesarean Delivery and Risk of Intestinal Bacterial Infection
The Journal of Infectious Diseases, Volume 0, Issue 0, Page 000, Latest Articles. Background. An individual’s intestinal bacterial flora is established soon after birth. Delivery by Cesarean section (c‐section) deprives the newborn of colonization with maternal vaginal bacteria. We determined whether delivery by c‐section is associated with an altered risk of infection with intestinal bacterial pathogens. Methods. In a cohort of 1.7 million Danes born 1973–2005 we identified cases of laboratory‐confirmed non‐typhoidal Salmonella species, Campylobacter species, Yersinia enterocolitica, Shigella species, and Shiga toxin–producing Escherichia coli from 1991–2005 in the National Registry of Enteric Pathogens. Using Poisson‐regression we estimated confounder‐adju...... MORE...

Successful treatment of severe retro-peritoneal bleeding with recombinant factor VIIa in women with placenta percreta invading into the left broad ligament: Unusual repeated ante-partum intra-abdominal bleeding
We experienced unusual repeated ante-partum intra-abdominal bleedings in a woman with placenta percreta invading into the left broad ligament, in whom recombinant factor VIIa was effective for controlling severe retro-peritoneal bleeding. A 34-year-old multiparaous woman with two prior cesarean sections presented acute abdominal pain at 16 weeks of gestation. She also represented with abdominal pain at 22 weeks of gestation, and a diagnostic laparoscopy revealed hemoperitoneum, although we could not identify the exact source of bleeding. She also represented with abdominal pain without vaginal bleeding at 27 weeks of gestation, and an MRI revealed a hematoma in the lower abdomino-pelvic region and placenta percreta invading into the left broad ligament. A classical cesarean section followe...... MORE...

Haiti: New life amid destruction
After the earthquake, eighteen-year old Djenny was one of the first women to deliver in Isaie Jeanty hospital in the Chancerelle area of Port-Au-Prince. Assisted by the Médecins Sans Frontières (MSF) team, Djenny gave birth to a healthy little boy that she named Mike. Many others have since joined them in the now bustling maternity ward. Before the earthquake, Isaie Jeanty hospital specialized in obstetric and maternity care. However, in order to meet the enormous surgical needs, MSF, working together with staff from the Ministry of Health, began to offer a wider range of services, including orthopedic surgery, post-operative care, physiotherapy and mental health support.  Yet, Isaie Jeanty continued to remain a referral centre for maternal care, with MSF helping to deliver an aver...... Effects of epidural fentanyl on speed and quality of block for emergency cesarean section in extending continuous epidural labor analgesia using ropivacaine and fentanyl.
Authors: Hong JY, Jee YS, Jeong HJ, Song Y, Kil HK We performed a prospective, randomized, and double-blind study comparing the top-up effects of 2% lidocaine/100 microg fentanyl/epinephrine (n=31) and 2% lidocaine/saline/epinephrine (n=30) when extending an epidural labor analgesia using low-dose ropivacaine and fentanyl. Survival analysis for the sensory blocks to the T4 level showed no statistically significant differences in onset time to T4 between the 2 groups. Onset times (min) to T4-sensory blocks for cold and pinprick were not different between the two groups. However, median maximum sensory level in the lidocaine-fentanyl group (T1 for cold and T2 for pinprick) was significantly higher than that in the lidocaine-saline group (T3 and T4, respectively). The lidocaine-fentanyl g...... MORE...

Attributable Costs of Surgical Site Infection and Endometritis after Low Transverse Cesarean Delivery •
Conclusions. The costs of SSI and EMM were lower than SSI costs reported after more extensive operations. The attributable costs of EMM calculated by the 2 methods were very similar, whereas the costs of SSI calculated by propensity score matched‐pairs were lower than the costs calculated by GLS. The difference in costs determined by the 2 methods needs to be considered by investigators who are performing cost analyses of hospital‐acquired infections. A prospective study of the effect of delivery type on neonatal weight gain pattern in exclusively breastfed neonates born in Shiraz, Iran
Conclusions: Type of delivery contributes strongly to the weight gain pattern in the first month of infancy. In spite of greater weight loss among C-section birth neonates in the first days of life, at the end of the first month neonates showed a similar weight gain. Consequently, mothers with C-section delivery can successfully exclusively breastfeed. (Source: International Breastfeeding Journal)... MORE...

Antibiotic prophylaxis versus no prophylaxis for preventing infection after cesarean section
Source: Cochrane Library Area: Evidence > Drug Class Focused Reviews Background The single most important risk factor for postpartum maternal infection is cesarean section. Routine prophylaxis with antibiotics may reduce this risk and should be assessed in terms of benefits and harms.   Objectives To assess the effects of prophylactic antibiotics compared with no prophylactic antibiotics on infectious complications in women undergoing cesarean section.   Search strategy We searched the Cochrane Pregnancy and Childbirth Group's Trials Register (May 2009).   Selection criteria Randomized controlled trials (RCTs) and quasi-RCTs comparing the effects of prophylactic antibiotics versus no treatment in women undergoing cesarean section.   Data collection and ...... Successful management of a patient with Marfan syndrome complicated with acute aortic dissection using landiolol during Cesarean section
Abstract  Aortic dissection is a lethal complication in pregnant women with Marfan syndrome. To decrease arterial wall stress, beta-blockers have been used as standard treatment, although uterine contractions caused by beta-2-adrenergic receptor antagonism may result. Herein, we report a patient with Marfan syndrome who was given landiolol, a selective beta-1-adrenergic receptor antagonist with a short half-life, for management during a Cesarean section procedure following development of acute aortic dissection. A 30-year-old pregnant woman with Marfan syndrome in the 38th week of gestation was referred to our department for an emergency Cesarean section because of development of acute aortic dissection. Blood pressure (BP) decreased from 157/70 to 128/64 mmHg after gi...... MORE...

Assessment of Emergency and Essential Surgical Providers at the District Hospital in Ghana
Conclusions: The present study demonstrates that there is a significant gap in education and training as well as an unmet need of human resources to provide surgical care in Ghana. These findings are likely to apply for other countries in Africa as well. Solutions include the development of formal short-term surgical training for MOs as well as exploring the training and utilization of physician extenders to support MOs when inadequate numbers are present. These data contribute to increasing evidence that the improvement of basic surgical and obstetrical services at the district hospital level must be the focus of future global health efforts. These data also underscore that importance of emergency and essential surgical care at the first referral facility as a necessary component of prima...... MORE...
