The Role of Whole-Body Imaging in the Diagnosis, Staging, and Follow-Up of Multiple Myeloma
Semin Musculoskelet Radiol 2010; 14: 037-046DOI: 10.1055/s-0030-1248705ABSTRACTMultiple myeloma is one of the most common hematological malignancies and accounts for significant morbidity and mortality. In the past, detection of myelomatous deposits by radiography was the main method of radiological assessment in this disease. However, in recent years the introduction of whole-body imaging techniques, specifically low-dose whole-body computed tomography, whole-body magnetic resonance imaging, and positron emission tomography, has facilitated a more comprehensive assessment of this heterogeneous and often diffuse disease. Each of these whole-body imaging techniques has specific applications and limitations in the assessment of multiple myeloma that the interpreting radiologist and referring......

Cerebrospinal Fluid Leaks of Temporal Bone Origin: Selection of Surgical Approach
Skull BaseDOI: 10.1055/s-0030-1249249ABSTRACTCerebrospinal fluid leaks of the temporal bone are rare, often occult, and sometimes challenging to localize and repair. This is a retrospective study of eight patients with spontaneous cerebrospinal fluid leak and six patients with cerebrospinal fluid leak or encephalocele discovered during chronic ear surgery who were treated in a tertiary medical center over a 5-year period. All received preoperative temporal bone computed tomography, and six also underwent magnetic resonance imaging, one computed tomography cisternography, and one radionuclide cisternography. All patients initially underwent a transmastoid surgical approach. Additional exposure was necessary in three patients; two underwent middle fossa craniotomy and another required minicr......

Primary Hodgkin's disease of the thymus
We report a 25-year-old man referred to us because of chest pain and irritating cough since 2 months. Physical examination was normal and no lymph nodes were palpable. Chest radiography showed an anterior mediastinal mass. Computed tomography (CT) examination of the chest revealed a homogeneous tissular mass occupying the anterior mediastinum in its upper and medium stage, extending to the infundibulum without lymph node enlargement. Thymectomy and wedge were performed via a median sternotomy. Pathologic diagnosis was nodular sclerosis Hodgkin's disease of the thymus. The patient received 6 cycles of ABVD (Adriamycin [doxorubicin], bleomycin, vinblastin and dacarbazine) and mediastinal radiotherapy. Currently, tow years after diagnosis, he is well and free of disease.Primary thymic Hodgkin......

Churg–Strauss syndrome diagnosed after lobar atelectasis: Case report
Abstract: A 24-year-old women presented with unresolved right upper lobe consolidation. Her medical history was notable for asthma, coronary artery disease, cerebrovascular accident, and nasal polyposis. Thoracic computed tomography scan revealed total atelectasis in the right upper and middle lobes, and consolidation in the atelectatic regions. A bronchial mucosal biopsy revealed diffuse eosinophilic infiltrates in the interstitium. Clinical presentation and pathological results of bronchoscopy were consistent with Churg–Strauss syndrome. In Churg–Strauss syndrome, radiologic pulmonary manifestations vary and atelectasis is very rare. In this case, mechanism of the lobar atelectasis might be explained by eosinophilic infiltration of airway walls resulting in airway narrowing. (Source:......

The right operation for the wrong reason in multi-focal non-small cell lung cancer
Abstract: In a sixty-seven year old female smoker, a right lower lobe mass, intimately related to the mediastinum, was detected on computed tomography (CT) scanning. Bronchoscopy confirmed non-small cell lung cancer (NSCLC) and a subsequent integrated 18F-FDG Positron Emission Tomography (PET)/CT scan demonstrated a rim of non-FDG avid tissue between the lung tumour and the mediastinum, and no nodal or distant metastases. The patient was pre-operatively staged as cT3N0M0 and proceeded to exploratory thoracotomy. On histological examination of the resected right lower lobe, multiple deposits of bronchioalveolar cell carcinoma (BAC) were found surrounding a central adenocarcinoma, in addition to a small deposit of adenocarcinoma in a right upper lobe wedge resection sample. Neither the BAC de......

Chapter 8 - Fungal infections in immunocompromised patients

Outcome of coronary plaque burden: a 10-year follow-up of aggressive medical management
Background:
: The effect of aggressive medical therapy on quantitative coronary plaque burden is not generally known, especially in ethnic Chinese. AIMS: We reasoned that Cardiac CT could conveniently quantify early coronary atherosclerosis in our patient population, and hypothesized that serial observation could differentiate the efficacy of aggressive medical therapy regarding progression and regression of the atherosclerotic process, as well as evaluating the additional impact of life-style modification and the relative effects of the application of statin therapy.MethodS: We employed a standardized Cardiac CT protocol to serially scan 113 westernized Hong Kong Chinese individuals (64 men and 49 women) with chest pain and positive coronary risk factors. In all cases included for this se......

High-resolution ex vivo magnetic resonance angiography: a feasibility study on biological and medical tissues
Conclusions:
Ex vivo MRA provides high-resolution of fresh tissue and delineates fine structures that we were not able to visualise using CT. We found that the use of our clinical imaging systems provided MRA which took advantage over CT angiography in its ability to visualize vessel configuration while avoiding interfering signals from adjacent bones. Interestingly, we found that vascular tissue becomes leaky when formalin-fixed, leading to increased permeability and extracellular leakage of MR contrast agent. Distinguishing Pancreatic Cancer from Autoimmune Pancreatitis
Abstract Both autoimmune pancreatitis (AIP) and pancreatic cancer frequently present with obstructive jaundice. However, AIP is a rare
disease and its diagnosis carries vastly different therapeutic and prognostic implications compared with that of pancreatic
cancer. The clinical challenge is to distinguish AIP from pancreatic cancer, because the price of misdiagnosis can be heavy.
Recently, two strategies for differentiating AIP from pancreatic cancer were published, one from Japan and the other from
the United States. The Japanese strategy relies on cross-sectional imaging, endoscopic retrograde pancreatogram, and serum
IgG4. The American strategy uses imaging (CT scan), serology (serum IgG4), and evidence of other organ involvement (on CT
scan) as the first tier of tests......

Finite element analysis of idealised unit cell cancellous structure based on morphological indices of cancellous bone
In this study, two idealised architectures were developed based on morphological indices of cancellous bone: the tetrakaidecahedral
and the prismatic. The two architectures were further subdivided into two types of microstructure, the first consists of struts
only and the second consists of a combination of plates and struts. The microstructures were transformed into finite element
models and displacement boundary condition was applied to all four idealised cancellous models with periodic boundary conditions.
Eight unit cells extracted from the actual cancellous bone obtained from micro-computed tomography were also analysed with
the same boundary conditions. Young’s modulus values were calculated and comparison was made between the idealised and real
cancellous structures. Results......
