Diagnostic and Surgical Imaging Anatomy: Ultrasound
by A. T. Ahuja et al, as the title suggests, is the ultrasound component in the Amirsys collection of textbooks for viewing, understanding, and, in this case, performing anatomical imaging. As with the entire series, the Ultrasound installment continues the excellence in image quality with the expanse of images for which they are known. In addition to the outstanding pictorial drawings, another hallmark of the Amirsys collection, there are also appropriate planar selections from computed tomography or magnetic resonance images providing further correlation to the sonographic images. The authors also take into account the unique imaging prospective ultrasound allows as an operator dependent modality; therefore, each image contains an adjacent schematic display of the orientation and placem......

Thresholds to Lung Density Measures
This article highlights an important topic for emphysema measurement. However, in their paper, the authors state, “Based on pathology comparisons…and literature-based consensus…−910 HU [Hounsfield units] was specified a priori as the threshold for primary analyses.” A threshold of −910 HU was established by Müller et al , who used one single-slice computed tomographic scan with a slice thickness of 10 mm and intravenous contrast. Slice thickness and the use of intravenous contrast influence lung density and the measurement of emphysema . Recently, Madani et al reported a study using multidetector computed tomography without iodinated contrast. The strongest correlation of multidetector computed tomography with macroscopy was observed at a threshold of −970 HU (r = 0.543, P <......

Drs Hoffman and Barr Replies:
We would like to thank Drs. Hochhegger, Irion and Marchiori for their kind comments and their thoughtful questions. The issue regarding appropriate density-mask thresholds for the characterization of emphysema via quantitative computed tomography (CT)-based assessment of the lung is an important one. Quantitative CT has been used with increasing frequency to assess presence, extent, and location of emphysema-like lung for the purposes of pharmaceutical efficacy testing, safety testing, population selection for device placement, and for the identification of homogeneous subphenotypes within the chronic obstructive pulmonary disease (COPD) population to better target new therapeutic development efforts. Although quantitative CT has contributed greatly to these efforts, the measures are, in ......

American Society for Radiation Oncology Announces Patient Protection Plan
The American Society for Radiation Oncology (ASTRO) announced a six-point patient protection on February 3, 2010. The plan was described as the culmination of a long-term effort to assure that patients receive safe and effective radiation therapy, an effort that continued at the ASTRO Board of Directors winter meeting on January 28-31, 2010.
The ASTRO plan includes the following recommendations: 1) creating a database for the reporting of linear accelerator- and computed tomography-based medical errors, 2) launching an enhanced practice accreditation program, specifically addressing new, advanced technologies such as IMRT, SBRT, and brachytherapy, 3) expanding educational programs, with special emphasis on quality assurance and safety, 4) working with patient support organizations to dev......

A case of Serratia granuloma in the soft tissue around the left kidney: a role of PTHrP in the formation of Serratia granuloma
Abstract
Serratia marcescens is an ubiquitous, saprophytic gram-negative bacillus that is associated with infections such as bacteremia, pneumonia and
osteomyelitis. However, it has not been known to form granulomas. A 72-year-old man with a history of tricuspidal insufficiency,
mitral insufficiency and ureterolithiasis presented with lumbago on the left side. He was admitted to our hospital, where
abscess formation in the subcapsular space and perirenal fat space of the left kidney, and left renal calculi were identified
by computed tomography of the abdomen. As infection and/or a tumor were suspected, nephrectomy was performed. The histopathological
findings in the resected kidney indicated severe infiltration by inflammatory cells with lymphoid follicles in the interst......

Remote cerebral hematomas in patients treated with intravenous rt-PA
We report our experience in patients treated with intravenous rt-PA. We reviewed our database of consecutive patients
treated at our hospital from 1999 to 2008. We used the inclusion/exclusion criteria of the ECASS-2 study from 1999 to 2003,
and the criteria of the SITS-MOST study since 2004. A follow-up CT scan was obtained in all of the patients within the first
36 h of treatment. Cerebral hemorrhagic complications were classified as hemorrhagic infarction (HI-1/HI-2) and parenchymal
hematoma (PH-1/PH-2). The rPH was defined as any extra-ischemic hemorrhagic lesion observed in the follow-up CT. A favorable
outcome was defined as a score 0–1 on the Rankin scale at 3 months. We treated 210 patients (mean age 67.6 ± 12.4 years, 56%
were men). The median ini......

Double-bundle ACL reconstruction: how big is the learning curve?
Abstract The study aim was to determine whether an experienced ACL surgeon could convert from a single-bundle to a double-bundle technique
with relative accuracy. We also wanted to determine whether there was a significant learning curve. Ten double-bundle ACL
reconstruction procedures were carried out on suitable individuals. Following the procedure, all patients underwent a CT scan
of the relevant knee. Femoral and tibial tunnel locations were then measured and compared to reference anatomical locations
previously described in the literature. The results were not known to the surgeon until all 10 cases were completed. The total
percentage difference between the sum of all four study tunnel locations from their reference anatomical positions was calculated
for each patien......

The development of a guidance level for patient dose for CT examinations in Korea
The primary goal of this study was to analyse the state of patient doses in the field of computed tomography (CT) examinations in the Republic of Korea. All survey data including the CT applications and patient dose details were obtained from general hospitals registered in the Korean Hospital Association. The systematic analysis of the patient dose survey gives target values of the weighted computed tomography dose index (CTDIw) and the dose–length product (DLP). The targeted CTDIw values were 69, 69, 31, 19, 44, 25, 24, 20, 2, 19 and 19 mGy for the brain non-contrast enhancement (BNCE), brain contrast enhancement (BCE), neck, chest, spine, liver, pancreas, stomach, kidneys, abdomen routine and abdomen trauma protocols, respectively. The targeted DLP values were 1056, 2112, 762, 123......

Free-hand thoracic pedicle screws placed by neurosurgery residents: a CT analysis
Abstract Free-hand thoracic pedicle screw placement is becoming more prevalent within neurosurgery residency training programs. This
technique implements anatomic landmarks and tactile palpation without fluoroscopy or navigation to place thoracic pedicle
screws. Because this technique is performed by surgeons in training, we wished to analyze the rate at which these screws were
properly placed by residents by retrospectively reviewing the accuracy of resident-placed free-hand thoracic pedicle screws
using computed tomography imaging. A total of 268 resident-placed thoracic pedicle screws was analyzed using axial computed
tomography by an independent attending neuroradiologist. Eighty-five percent of the screws were completely within the pedicle
and that 15% of the screws v......
