Breast MRI After Conservation Therapy: Usual Findings in Routine Follow-Up Examinations
CONCLUSION. After a patient has undergone BCT, MRI shows changes in both breasts. Although the changes in our study population were greatest in the treated breast, parenchymal enhancement and cystic alteration decrease bilaterally indicating a systemic influence. Edematous changes, seroma, focal enhancement, and skin thickening were seen only in the treated breast. All posttreatment MRI findings decrease progressively, and all may persist. Lumpectomy site enhancement is most persistent in women with fat necrosis. (Source: American Journal of Roentgenology)... MORE...

Breast cancer in young women
Abstract  Recent evidence suggests that the incidence of local breast tumor recurrences and failures may be substantially lower than previously thought due to the widespread use of systemic adjuvant therapy and loco-regional treatments. Surgery is a fundamental aspect of the management of breast cancer, and the mainstay of loco-regional treatment. In younger women, the choice of surgical approach (radical mastectomy vs lumpectomy) is influenced by tumor size, multicentricity, breast volume, genetic predisposition to the disease, and the patient’s wishes, because breast resections can have a major impact on the patient’s quality of life. Currently, adjuvant radiotherapy is recommended for all women following breast surgery. It significantly reduces the rate of relapse an...... MORE...

Breast Reduction in the Irradiated Breast: Evidence for the Role of Breast Reduction at the Time of Lumpectomy
Abstract: Given the high incidence of breast cancer in our society, it is common to encounter patients with macromastia who desire breast reduction after breast-conserving therapy by lumpectomy and radiation. We hypothesize that radiation leads to a significant increase in postoperative complications after breast reduction. All patients with a history of unilateral breast lumpectomy and radiation who subsequently underwent bilateral breast reduction by a single surgeon from 2004 to 2008 were retrospectively reviewed. Outcomes including cellulitis, wound breakdown, seroma, and need for repeat operations were compared between the radiated and nonradiated breast. The Fisher's exact test was used for statistical analysis. Twelve patients (mean age, 57 years) underwent bilateral breast reductio...... MORE...

Outcome after mastectomy for ipsilateral breast tumor recurrence after breast conserving surgery.
Authors: Beard HR, Cantrell EF, Russell GB, Howard-McNatt M, Shen P, Levine EA Ipsilateral breast tumor recurrence (IBTR) is a risk after breast conserving surgery, and is traditionally treated with mastectomy. Given the limited literature on outcome after mastectomy for IBTR, we evaluated our long-term data for this group. A retrospective review was conducted using a database of 2101 breast cancer patients at a single institution. Fifty-nine patients underwent breast conserving surgery and experienced an IBTR. Exclusion criteria included repeat lumpectomy or metastatic disease before mastectomy. Patients presented with invasive ductal (58%), invasive lobular (7%), other invasive (11%), or ductal carcinoma in situ (24%). Initial tumors were Tis (24%), T1 (42%), T2 (20%), T3 (2%), or no...... Reduction mammaplasty as a secondary therapy option for chronic pain after lumpectomy in the breast: a case report
Abstract  Chronic breast pain after surgery can have a profoundly negative impact on the physical and psychosocial function of patients. Treatment options are still in development, and no method hitherto described 100% pain relief. A 45-year-old woman suffering from chronic pain following breast surgery, who underwent reduction mammaplasty that lead to 100% pain relief, is presented. We suggest that mammaplasty could be a valuable treatment option in treating carefully selected patients with chronic breast pain. To our knowledge, this is the first report of mammaplasty for the treatment of chronic breast pain following breast surgery. Dosimetry evaluation of SAVI-based HDR brachytherapy for partial breast irradiation
Sivasubramanian R Manoharan, R Rodney Rodriguez, Vidya S Bobba, Mukka ChandrashekarJournal of Medical Physics 2010 35(3):131-136Accelerated partial breast irradiation (APBI) with high dose rate (HDR) brachytherapy offers an excellent compact course of radiation due to its limited number of fractions for early-stage carcinoma of breast. One of the recent devices is SAVI (strut-adjusted volume implant), which has 6, 8 or 10 peripheral source channels with one center channel. Each channel can be differentially loaded. This paper focuses on the treatment planning, dosimetry and quality assurance aspects of HDR brachytherapy implant with GammaMed Plus HDR afterloader unit. The accelerated PBI balloon devices normally inflate above 35 cc range, and hence these balloon type devices cannot be acco...... MORE...

Update on DCIS Outcomes from the American Society of Breast Surgeons Accelerated Partial Breast Irradiation Registry Trial
Conclusions  During an extended follow-up period, APBI delivered via MammoSite continued to be well tolerated for patients with DCIS. Use of this device may make lumpectomy possible for patients who would otherwise choose mastectomy because of barriers associated with standard radiation therapy. Clinicopathologic Presentation of Asian-Indian American (AIA) Women with Stage 0, I & II Breast Cancer
Abstract  Although numerous studies have looked at cancer incidence and survival in Asian Indian-American (AIA) patients, there is a paucity of data regarding clinicopathologic presentation of cancer in this ethnically diverse population. After receiving IRB approval, AIA patients of Indian and Pakistani descent who presented with Stage 0, I, & II breast cancer to our facility were identified. Charts were extracted for clinical and pathologic variables in addition to outcomes data. Standard statistical analyses were performed using SAS (v 9.1). The population (n = 50) consisted of 86% Indian (n = 43) and 14% Pakistani (n = 7). The median age at diagnosis was 52 (range 25–79). Sixty-three percent of tumors were detected after discovery of a pal...... MORE...

Prone versus supine positioning for whole and partial-breast radiotherapy: A comparison of non-target tissue dosimetry
Conclusions: In the context of tangential-field WBI and PBI, prone positioning is likely to benefit left-breast-affected women of larger breast volume, but to be detrimental in left-breast-affected women of smaller breast volume. Right-breast-affected women are likely to benefit from prone positioning regardless of breast volume. (Source: Radiotherapy and Oncology)... MORE...

Clinical Series of Oncoplastic Mastopexy to Optimize Cosmesis of Large-Volume Resections for Breast Conservation
Conclusions  Oncoplastic mastopexy allows the surgeon to address large tumors or tumors in cosmetically difficult sites adequately for breast conservation. Careful margin marking and re-excision of close or positive margins is still often feasible to achieve adequate negative margin with acceptable cosmesis in spite of the large initial volumes of resection. The Effect of Simultaneous Peripheral Excision in Breast Conservation upon Margin Status
Conclusions  Smaller tumor size, lack of EIC, and greater numbers of simultaneous PSs excised decrease the likelihood of positive margins, despite a lack of correlation between segment numbers and excised volume. These findings suggest that excision of simultaneous PSs may assist in achieving negative margins, in part, because of avoidance of pathologic artifact.
